Writing a Suspense/thriller

Writing a thriller by definition has the main character constantly in and out of danger.  But how does the main character get out of those jams?  If it’s by sheer stupidity or seems more like the author rescuing them, how’d that character survive long enough to be a main character?  How about the villains?  Do they treat the main character differently when they have the chance to kill them off or for some reason, they always hold off whereas they kill off ancillary characters quickly and expediently?  More in this video.  If you’re a writer, what about it?  If a reader, what do you think about it?
